Doctor Explains: Can Resonance Frequency Breathing Help Brain Fog in ME/CFS, Long COVID and fibromyalgia?

Brain fog is one of the most frustrating symptoms experienced by people with ME/CFS, Long COVID and fibromyalgia. Many patients describe feeling as though they are "thinking through treacle"—finding it difficult to concentrate, remember information, process conversations, or make decisions.
As an ME/CFS specialist, one question I am increasingly asked is whether resonance frequency breathing can improve brain fog. The technique has become popular on social media and wellness websites, with claims that it can calm the nervous system, improve concentration and even reduce fatigue.
But what does the evidence actually show?
What is resonance frequency breathing?
Resonance frequency breathing is a slow breathing technique, usually involving around five to six breaths per minute. The goal is to synchronise your breathing with natural changes in heart rate, improving something called heart rate variability (HRV).
Heart rate variability measures the tiny differences in time between each heartbeat. In healthy people, a higher HRV generally reflects better balance between the sympathetic ("fight or flight") and parasympathetic ("rest and digest") nervous systems.
Many researchers believe that improving this balance may help reduce stress and support better cognitive performance.
Why is this relevant to ME/CFS?
One of the most active areas of ME/CFS research involves the autonomic nervous system.
Many people with ME/CFS have evidence of autonomic dysfunction, including problems such as dizziness on standing, rapid heart rate, poor temperature regulation and exercise intolerance. Conditions such as POTS (Postural Orthostatic Tachycardia Syndrome) frequently overlap with ME/CFS and Long COVID.
Researchers have also found that many patients have reduced heart rate variability compared with healthy individuals, suggesting reduced parasympathetic activity.
Because resonance frequency breathing appears to improve heart rate variability, it has attracted interest as a possible way to reduce symptoms such as brain fog.
What does the research show?
One of the best-known studies is "Effect of Resonance Breathing on Heart Rate Variability and Cognitive Functions in Young Adults: A Randomised Controlled Study."
Researchers recruited 50 healthy young men aged between 18 and 30 years. Half practised resonance frequency breathing for 20 minutes each day over four weeks, while the control group continued their normal routine.
At the end of the study, those performing resonance breathing demonstrated:
improved heart rate variability
reduced perceived stress
better attention
faster performance on cognitive tests measuring executive function
increased activity of the parasympathetic nervous system.
These findings suggest that slow breathing can positively influence both autonomic function and certain aspects of thinking and concentration.
Does this mean it works for ME/CFS brain fog?
This is where it is important not to over-interpret the findings.
The participants were healthy volunteers, not people living with ME/CFS, Long COVID or fibromyalgia.
Brain fog in ME/CFS is likely to result from several overlapping mechanisms, including altered brain blood flow, neuroinflammation, abnormal energy metabolism, autonomic dysfunction, poor sleep and post-exertional malaise.
Improving heart rate variability alone may not be enough to overcome all of these factors.
At present, there are no high-quality clinical trials showing that resonance frequency breathing improves cognitive symptoms specifically in people with ME/CFS.
However, the study provides a reasonable scientific basis for investigating the technique further.
Could it still help some patients?
Possibly.
One advantage of resonance frequency breathing is that it is:
free
easy to learn
generally safe
supported by evidence for reducing stress in healthy adults.
Many patients report that slow breathing helps them feel calmer, improves sleep quality or reduces feelings of being "overstimulated." Some also notice clearer thinking after regular practice.
These individual experiences are valuable, but they should not be confused with proof that the technique treats the underlying illness.
Like many aspects of ME/CFS therapy, breathing exercises may benefit some people more than others.
How should you practise it?
The aim is relaxed, comfortable breathing rather than taking deep breaths.
A simple approach is:
breathe in gently through your nose for approximately five seconds
breathe out slowly for approximately five seconds
allow your breathing to remain effortless
keep your shoulders relaxed
start with five minutes daily and gradually increase only if well tolerated.
Many smartphone breathing apps or visual breathing timers can help maintain a comfortable rhythm.
Importantly, people with ME/CFS should avoid pushing through symptoms. If a 20-minute breathing session leaves you more fatigued or triggers post-exertional malaise, the session is too long.
Is it safe?
For most people, yes.
However, breathing should never feel forced. Taking excessively deep breaths can lead to hyperventilation, causing dizziness, tingling or light-headedness.
Patients with severe ME/CFS should also remember that even gentle activities can consume limited energy. Resonance breathing should fit comfortably within your daily energy envelope rather than becoming another task to complete.
Where does resonance breathing fit into treatment?
Breathing exercises should be considered one possible supportive strategy, not a cure for ME/CFS.
Good management usually begins with an accurate diagnosis and appropriate ME/CFS testing to exclude other causes of fatigue and identify associated conditions that may be contributing to symptoms.
Individualised ME/CFS therapy may include pacing, management of orthostatic intolerance, sleep optimisation, nutritional advice, symptom-targeted medication and treatment of overlapping conditions.

The bottom line
Resonance frequency breathing is an interesting area of ME/CFS research. A well-designed randomised study found that it improved heart rate variability, reduced stress and enhanced certain aspects of cognitive performance in healthy adults.
However, we do not yet have evidence that it specifically improves brain fog in people with ME/CFS.
That said, because it is inexpensive, generally safe and simple to practise, some patients may find it a useful addition to an overall symptom-management plan. The key is to view it as one tool among many rather than a standalone treatment.
